Questions, Answers, and Information Directly from Dr. Pamela Brownlee, DO

Sourced directly from Dr. Pamela Brownlee, DO's social posts and replies.

When is fat grafting preferred over implants for breast augmentation, and vice-versa?
Fat grafting to the breast is suitable for individuals desiring a very subtle or small change, or for patients undergoing a lift who want more upper pole fullness but do not want implants. Implants, conversely, are recommended for those seeking a significantly larger cup size, a substantial change in size, or very firm upper pole fullness, as they allow for a much larger increase in size compared to fat grafting.
What are the realities of fat grafting, including fat survival and permanence?
Dr. Brownlee informs patients that a certain percentage of implanted fat will not survive, typically 20-40%, with her usually advising around 30%. This fat loss occurs regardless of patient adherence to post-op rules. To compensate, the area is usually slightly overfilled during the procedure. The fat that remains after grafting is permanent, develops blood vessels, is alive, and will gain and lose weight along with the patient's body. It's crucial that fat is injected in thin layers, like icing on a cake, to ensure blood flow and survival; improper injection can lead to oil cysts or hard, painful masses.
Can liposuction be performed on the central abdomen during a tummy tuck?
Dr. Brownlee does not perform liposuction on the front central abdomen simultaneously with a tummy tuck due to the risk of decreased blood flow to the incision edges. This could lead to a large, slow-healing wound instead of a thin scar. However, if a patient carries weight in this area, liposuction can be performed approximately six months after the tummy tuck, once blood flow is restored and the incision is well healed.
What are Dr. Brownlee's common applications for Botox, including non-cosmetic uses?
Dr. Brownlee uses Botox to address various patient concerns, including forehead lines, frown lines (corrugator), crow's feet, bunny lines on the nose, and chin wrinkles (mentalis). She also personally uses Botox for therapeutic purposes, such as in her left trap muscle to alleviate shoulder pain from operating and in her masseter muscles to treat teeth grinding and prevent cracked teeth and tension headaches. She emphasizes that Botox dosage is highly individualized and determined collaboratively with the patient.
